Additional Documents Required
The documents to be submitted as proof for address change are (Any 2 proof one should be Original, one should be photo ID)
(a) Current Passport in original with photocopy (of first, last, address page (if separate page), any other endorsement pages, visa page and additional booklets, if any. For minors (up to 18 years) parents' passport copies and visa copy are required
Proof of Address (Any One): For all Adults
Recent & Original Electricity / Telephone/ Water / Gas Bills (in English) not older than three months.
Election Commission Card / e-Aadhar / Aadhar Card to be produced in ORIGINAL at the time of submitting the application.
Original Nativity Certificate attested by the concerned state home department / online nativity certificate which can be verified online.
Minors applicants --- As per parent's passport ONLY.
A passport expired for more than six months should be supported by duly attested Sworn Affidavit explaining the circumstances and the reasons for late renewal.
Personal appearance of applicant is compulsory to establish physical identity.
Any Changes to be amended in the passport should be accompanied with request letter & documentary evidence.
