Additional Documents Required

• Current Passport in original with photocopy (of first, last, address page (if separate page), any other endorsement pages, visa page and additional booklets, if any.
• Spouse Passport copies photocopy (of first, last, address page if separate page, any other endorsement pages).
• Divorce Decree issued by a competent Court Divorce order from Wakaf Board Death Certificate issued by the Registrar of Deaths needs to be dully attested by Home Department of the concerned State Government in India. If the original divorce certificate is issued in a language other than English legal translation to be obtained through the original marriage certificate should have all the relevant attestation.
• Photocopy of husband's passport (original to be shown while submitting the application).
• Original and a copy of Marriage Certificate issued by Registrar of Marriages duly attested by the Home Department of the concerned State in India where the marriage is registered. If married abroad then the certificate needs to be attested from an Indian mission with jurisdiction for that country. If the original marriage certificate is issued in a language other than English legal translation to be obtained through the original marriage certificate should have all the relevant attestation.
• Name in Marriage Certificate should match with the passport without any spelling errors.
• Deed Poll/Sworn Affidavit, in the prescribed format, signed by applicant in the presence of Consular Officer for change of name.
• If changing the signature than letter, in the prescribed format, signed by the applicant in the presence of the consular officer.
• Personal appearance of applicant is compulsory to establish physical identity.
• Any Changes to be amended in the passport should be accompanied with request letter & documentary evidence.
