Photocopy of the lost passport (of first, last, address page if separate page, any other endorsement pages and visa page with additional booklets, if any. For minors (up to 18 years) parent’s passport original for verification and copies are required.
If stamped UAE Residence Visa page/passport copy is not available, prior approval from EOI is required
Copy of the First Information Report (FIR) from the Police authorities and its authentic legal English translation in original is required.

Any Changes to be amended in the passport should be accompanied with request letter & documentary evidence.
Applicants who are on visit visa to UAE will be issued an Emergency Certificate with prior approval from EOI to enable his/her return to India, except in exceptional cases.
