New passport in maiden name after obtaining passport in married name, consequent upon divorce/death of husband, in the case of ladies
Additional Documents Required
Old Passport /School Certificate/Birth Certificate/ Aadhar card /Voter ID copy. etc. any two document to be submitted which is holding maiden name.
The supporting photo ID or documents for name change should have the date of birth & father name matching as per the existing passport.
In case of Divorce, Divorce Decree required. If in a language other than English, Legal translation to be obtained.
In case of death case Death Certificate issued by the Registrar of Deaths needs to be duly attested by Home Department of the concerned State Government in India. If the original death certificate is issued in a language other than English, Legal translation to be obtained.
