Surrender of Indian Passport/Surrender Certificate

2 recent clear photos 51mm*51mm (not older than three months, no uniform) with white background and DARK DRESS with frontal view. Original Indian Passport (expired passports will also be accepted. Revoked, impounded and suspended passports will not be accepted for surrender)
Both parents original passport for verification in case of minor.
Photocopy of valid foreign passport, along with previous foreign passports if any, and photocopies of pages first two, last two and pages containing valid visa and other observations. (Photocopy services are available in the BLS center)
Citizenship Certificate/ Naturalization Certificate/Registration Certificate in English from the foreign Country, indicating the date of acquiring new nationality.
Nationality ID copy of the country acquired nationality.
Request letter from the parents on the reason for surrender of Indian Passport.
Address proof from UAE (DEWA bill / tenancy contract / Bank account statement / Affidavit)
(If any document is in any foreign language other than English, it should be translated in English by authorized translator and then attested by your Consulate/ Foreign Ministry).
The applicant should submit the application in person for identification and both PARENT’s should accompany the child.
Applicants (except minors) could be called for personal interview in Consulate General of India - Dubai which would be mandatory.
